If you need to change your child’s dismissal plan on any given day from pick-up to bus or bus to pick-up, or after-school activity, you may use this form. Please use a separate form for different options per student. Any changes must be submitted by 12:00pm on the day of change. After 12:00 pm we will NOT be able to accommodate any changes to your child’s dismissal plan unless it is an emergency.
